Off-the-shelf allograft

An off-the-shelf allograft is a donor-derived tissue product supplied pre-processed and ready to use, without requiring in-office harvesting or processing equipment.

The term describes a format, not a tissue type or a regulatory status. Unlike autologous preparations such as PRP, BMAC or adipose-derived tissue, which require harvesting and processing equipment at the time of the procedure, an off-the-shelf allograft arrives from the supplier already recovered, processed, tested and packaged for use.

This format shifts operational responsibility to the clinic in different ways than autologous options do. Instead of managing in-office processing equipment, the clinic manages inventory, including cold-chain monitoring where applicable, expiration dating, and lot segregation, while relying on the supplier's donor-eligibility and testing program rather than same-day in-office testing.

Because 'off-the-shelf' describes convenience rather than safety or performance, buyers evaluating this format should still request the supplier's full documentation chain for the specific lot being purchased, including donor eligibility, sterility testing results, lot release criteria and the certificate of analysis.

The convenience of an off-the-shelf format does not substitute for lot-specific documentation, so clinics should request the full paper trail for every lot they stock.
